The Blundellsands Surgery has put a re-call system into place, to ensure you are invited for one review per year for your conditions. Our aim is to invite you to attend around your Birthday Month to act as a reminder for you too.

If you have any concerns about your review being overdue please contact the surgery.


You will be re-called if you have one or more of the following conditions:

Atrial Fibrillation (AF)           

Coronary Heart Disease (CHD)

Chronic Kidney Disease (CKD)

Chronic Obstructive Pulmonary Disease (COPD)

Diabetes Mellitus


Heart Failure

Mental Health

Non-Diabetic Hyperglycaemia 

Peripheral Artery Disease (PAD)

Rheumatoid Arthritis



Please Note these reviews are in addition to any hospital clinics you attend for your conditions.

1. Your first invite will be via letter, telephone or text - you will be contacted a further time should you not respond to your first invite.

2. You will be asked to arrange a Blood Test appointment at the community clinics by using the following link: or by calling 0151 285 4548.

3. A Practice Nurse appointment will be arranged with you, for when your blood results are available. 

4. You will have your review BP, Height, Weight and Lifestyle will be reviewed - If during your review the nurse feels you need to be seen more frequently than annually you will be advised of this during your appointment i.e. those with Diabetes.

Your annual review is an extremely important part of your care and therefore it is imperative we have all of your details correct in order for us to contact you.
